CARE OF PEOPLE WITH CHRONIC ILLNESS: CARE PLANS

 

We know from the Department of Health that chronic illness is widespread in the community.

  • 1 in every 2 Australians have at least one prominent chronic condition- for example,  arthritis, asthma, back pain, cancer, cardiovascular disease, chronic obstructive pulmonary disease, diabetes or mental health conditions.

  • nearly a quarter of all Australians 60% pf those aged over 65 years, had two or more chronic conditions.

    When people have complex, chronic health conditions, there is a need for a comprehensive approach by your GP to ensure that nothing gets missed, that you get to see the right health professionals and that your health problems are regularly reviewed.

     

This is what Care Plans are designed to do.  A particular advantage of Care plans is that patients receive 5 subsidised visits to allied health (eg physio) over 12 months. 

One of our major priorities is care of people with chronic illness> if our doctors think you can benefit from a Care Plan, they will raise it with you as part of the normal consultation process
